Keystone HMO Plans

HMO plans are designed to help your employees save money. A participating primary care physician (PCP) coordinates all of their care.

  • In-network coverage
  • Participating PCP coordinates employees’ primary care and refers them to in-network specialists for specialty care
  • Emergency and urgent care access available outside the United States through Blue Cross Blue Shield Global Core®

Keystone POS and Direct POS Plans

POS and Direct POS plans provide both-in and out-of-network coverage. With a POS plan, members get the highest level of benefits by asking their PCP for a referral to specialists in network. With Direct POS, referrals are only needed for laboratory services, physical/occupational therapy, and spinal manipulation/chiropractic care.

  • Must select a participating PCP to coordinate care
  • POS requires referrals to specialists for the highest level of benefits
  • Direct POS requires referrals for only a few services
  • Away from Home Care® program for employees who temporarily reside outside the service area

Personal Choice® PPO Plans

PPO plans offer your employees flexibility to choose in-network or out-of-network providers. The choice is always theirs to make. Much higher out-of-pocket costs may apply for out-of-network care.

  • In- and out-of-network coverage
  • Freedom to see any doctor or specialist without referrals
  • In-network benefits available coast-to-coast through the BlueCard® PPO network and around the world through Blue Cross Blue Shield Global Core®

High-Deductible Health Plans (HDHPs)

Our HDHPs are PPO plans that help give your employees more control over planning and paying for qualified health care expenses.

  • Mobile access to balance information with the IBX app

Choice Advantage Plans

Choice Advantage plans include site of service benefits to help members save on out-of-pocket costs based on where they receive care. Members have the choice to save hundreds of dollars in some cases.

Members with Choice Advantage POS plans can save on preventive colonoscopies and outpatient surgery, while Choice Advantage PPO members can save on:

  • Lab/pathology
  • Physical/occupational therapy
  • Routine/complex radiology
  • Outpatient surgery
  • Preventive colonoscopy

For example, members can save on outpatient surgery by choosing an ambulatory surgery center instead of a hospital, or they can save by choosing LabCorp® instead of getting bloodwork at a hospital.

Personal Choice PPO Tiered Network Plans

PPO tiered network plans give members the option to save on hospital and outpatient surgery services. Acute care hospitals and facility providers are grouped into two in-network tiers, based on cost and quality measures. Members pay less when they use Tier 1 providers and more when they choose Tier 2 providers.

Members can save on a number of hospital and outpatient surgery services, including:

  • Outpatient surgery at ambulatory surgical centers
  • Inpatient hospital admissions for medical and maternity care
  • Hospital-based outpatient radiology centers
  • Hospital-based outpatient labs
